The Diabetes Get-Checked® Programme is now in its eighth year and is regarded as an important tool
by doctors and practice nurses, diabetes specialists, District Health Boards and the Ministry of Health in delivering high quality diabetes care in general practice. It is different from other screening programmes because its focus is on constraining disease progression in already diagnosed diabetics rather than identifying patients with previously undiagnosed diabetes. Through implementation of this plan, which commenced in 2006, Waikato Primary Health aims to improve health outcomes for Maori by increasing access to local primary health care services; ensuring Mâori participate in Waikato Primary Health planning and decision making; ensuring that health information on Mâori is collected and used to improve the health of Mâori and by improving the capacity of primary health care providers to deliver responsive services to Mâori. A second Changing Faces report has been released by Pinnacle’s Primary Health Intelligence Unit, which provides WPH with important insight into the representativeness of their enrolled population. At the time this report was prepared no data on deprivation level based on 2006 census was available.
By comparing WPH enrolled population with the 2006 census population data for the Waikato DHB area, it makes it possible to estimate with confidence social-demographic information about the community WPH serves to facilitate informed decision making about population health initiatives. Changing Faces 2 will greatly enhance the PHOs understanding of the factors affecting their populations need for and access to primary health services.
